Access/Assessment Center |

The
Access Center provides screening services for both involuntary
and voluntary clients, and is provided
on an in-house, walk-in basis for individuals seeking
care for themselves or a family member.

The Adult Case Management
program serves adults who exhibit severe impairment due
to a chronic and persistent mental illness and/or
co-occurring substance use disorder.

planning and linking to community resources. |

Units (CSU I & II) |
The Adult Crisis
Stabilization Units seek to reduce the need for
psychiatric hospitalization among those people
exhibiting acute mental health disorders by providing
intensive chemotherapeutic stabilization with ongoing
medical assessment and care, education, training and
social interactions.

Adult Medication Clinic |
|
The Adult Medication Clinic
provides psychiatric intervention through medication
management, education and nursing services, as well as
referrals to other services as needed. The length of
the program varies depending on the recommendations for
treatment.

Outpatient Program |
The Adult Outpatient
Program provides comprehensive, focused,
psychotherapeutic services to individuals on an
outpatient basis which may include individual or group
therapy sessions.

Most clients who qualify for
services are discharged within six months of

Children’s
Crisis Stabilization Unit (CCSU) |
The Children's Crisis
Stabilization Unit is a 20-bed inpatient facility
providing psychiatric services and crisis intervention
24 hours a day, 7 days a week for clients who meet Baker Act criteria. Services
are provided to children who exhibit symptoms of a
primary psychiatric disorder, are a danger
to themselves or others and cannot be stabilized in a
less restrictive setting.

The Children’s Medication
Clinic serves children and adolescents through the age of
17 with serious behavioral and emotional problems not
requiring inpatient care.

Children’s

Children’s Outpatient services
provide intensive therapeutic on-site and home based
services, social rehabilitation services, basic
living skills training, individual and group therapy to
children suffering from mental health, co-occurring
substance use disorders, and their families.

Lakeside’s Entitlement Services serves as a point
of entry for many individuals in urgent need of public
assistance to obtain much needed care. The goal of Entitlement
Services is to assist all self-pay clients in filing for benefits – including social security and state
assistance programs.

Florida
Assertive Community Treatment (FACT) Team
|
|
Lakeside's Florida Assertive Community Treatment
(FACT) Team is made up of a multidisciplinary, mobile
group of treatment providers including a psychiatrist,
psychiatric nurse, counselor, case managers and peer counselors.
Maintaining a small caseload, this group functions interchangeably
to provide treatment, rehabilitation and support services
that the person with mental illness
needs to live successfully in the community.

Forensic
and State Hospital Team
Community Competency Restoration
Program (CCR) |
The Forensic and State
Hospital Team provides evaluation, advocacy and linkages
for services in the community
for clients involved in either the forensic system or
state hospital system. The CCR also provides
education to the community and other providers related
to forensic clients and services.

Intensive
Team Case Management Services (ITCM)

The Intensive Case
Management Team provides
services and support to clients who suffer from severe
and persistent mental illness and/or co-occurring
substance use disorders, 24 hours a day, 7 days a week. The purpose of the program is to reduce the
need for psychiatric hospitalizations and to assist consumers
in obtaining placement in the least restrictive
community environment.

Clients are reassessed every six months and
services are extended up to one year as needed. |

The PATH
Case Management program serves clients who are homeless
and have mental health issues.
Outreach Specialists target individuals in facilities
such as the Coalition for the Homeless, Women’s Residential
Counseling Center (WRCC), Covenant House, O.U.R. Mission,
Anthony House, Salvation Army and meal sites. An
informal assessment is conducted regarding
the individual’s need for mental health treatment.
If the person is identified as needing treatment for
severe mental illness, they are entered into the
Lakeside system.

Residential
Program: Independence Place and College Place
|
Residential Plaza, comprised of Independence
Place, a 15-bed group home and College Place Apartments,
a 36-bed supervised apartment facility, provides
transitional housing as well as other services to
individuals with a mental illness. It is a point of
entry for adults, including the homeless, with severe
and persistent mental illnesses who need the support of
supervised living until they have regained the skills
necessary for more independent living.
